SUMMARY OF POSITION: Provides nursing case management/ delegating RN services within a group home/residential and community setting.

QUALIFICATIONS/ SKILLS

  • A graduate of an accredited School of Nursing and licensed by Maryland Board of Nursing as a Registered Nurse ( BSN preferred).

  • Current status with Maryland Board of Nursing as a Delegating Nurse

  • Comprehensive knowledge of DDA and COMAR regulations preferred

  • Two years’ experience supporting adults with developmental disability

  • Previous hospital or community nursing experience

  • MTTP Trainer experience preferred, but not required

  • CPR and First Aid certified.

  • Ability to travel locally

  • Good oral and written communication skills

  • Excellent nursing assessment skills and possess the ability work independently

  • Team player, Responsible, Accountable, Ethical and Flexible

CORE RESPONSIBILITIES:

  • Responsible for health and safety of Persons supported by EPIC.
  • Collaboration and communication with various health care professionals
  • Timely completion of 45-day nursing assessments and Individualized nursing care plans
  • Oversight of medical appointments
  • Assigns, monitor and follow-up with tasks given to the Medical Appointment Coordinator.

  • Provide Staff training to Direct Support staff on an ongoing basis
  • Ensure compliance with DDA, OHCQ, and OSHA regulations as well as Maryland Board of Nursing policies and procedures.
  • Acts as liaison between EPIC staff, Healthcare professionals, families, and other members of the interdisciplinary team.
  • Responsible for medical evaluations of persons supported as needed.
  • Acts as an advocate for persons supported by EPIC, ensure persons supported are treated with respect and dignity at all times.
  • Performs other duties as designated by the Director of Nursing.

SUPERVISORY ROLE:

  • Monitors delegated nursing tasks provided by CNA’s, CMT’s and Direct Support Professionals.
  • Performs regular site visits to assess health and safety compliance.

RECORD KEEPING & DOCUMENTATION:

  • Maintains persons supported confidence and protects operations by monitoring confidential information processing.
  • Performs the initial and ongoing health assessments for persons supported.
  • Completes and updates nursing care plans for assigned caseload.
  • Ensure individual’s health information is readily available for review by applicable parties.

Work Environment: This job operates in a professional environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Physical Demands: While performing the duties of this job, the employee is regularly required to talk and hear. Employee must occasionally lift and/or move items that weighs up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
