SUMMARY OF POSITION:

​​Community Based: 8:30 a.m. – 3: 30 p.m. ​; ​ Job Site:​ ​Community Based ​. As an essential employee, your attendance is required at work to perform essential job duties even in cases of inclement weather, holidays.

​​$18.00​ hour

​​Position requires​ ​35% of travel​​​

​​The position of CDS​​- ​The position of DSP in Community Development Service (CDS) provides direct support services to adult person(s) supported with intellectual challenges. The DSP supports the person(s) with community-based services that provide person supported with development and maintenance of skills related to community membership through engagement in community-based activities with people without disabilities. The DSP provides the person(s) with opportunities to develop skills and increase independence related to community integration, promote positive growth and developing general skills and social supports necessary to gain, retain, or advance competitive integrated employment opportunities, and personal care assistance, based on assessed need. 

  • ​​Must have a​ ​high school diploma or GED​.   

  • ​​Must have​ ​intermediate​ ​Microsoft​ ​Word​ ​skills.​ 

  • ​​Must possess a local, valid U.S. driver’s license with no more than 3 moving violations and/or at fault accidents in the past 3 years or any major violations in the past 5 years  

  • ​Must have a reliable vehicle with current registration and insurance that will be used to transport adult(S) in the community. ​ 

  • ​​ Must have the ability to successfully pass COMAR mandated training and basic skills tests ​

    • ​Attends company/department meetings, trainings, and events as required.​ 

    ​​ Direct Support Duties: 

    • ​Teaches person(s) supported vocational skills to help prepare them for community integration and provide instruction/supervision in the community. 

    • ​Teaches person(s) supported social/survival skills to help them lead more independent lives, including mobility, money management, housing, and public transportation. 

    • ​Supervises Person(s) supported during hours of instruction and on community outings. 

    • ​Builds friendly and professional rapport with Person(s) supported and act as a positive role model for positive habits, communication skills, and social skills. 

    • ​Assists in implementing person(s) supported support plans.  

    • ​Collectively, with the assistance of Group Supports leadership team, creates and implements activities for Person(s) supported to build their basic communication and social skills (staying in-line with their person(s) supported goals). 

    • ​Accompanies Person(s) supported on recreational and educational activities such as field trips, concerts, museums, etc. 

    • ​Maintains person(s) supported hygiene. 

    • ​Keeps person(s) supported(s) engaged in daily activity of choice. 

    • ​Prepares weekly schedules, monthly activity projections, and written progress reports for person(s) supported. 

    • ​Maintains constant supervision of the Person(s) supported(s) assigned. 

    • ​Instructs person(s) supported of their rights and that of others as per. Resource Connections/DDA. 

    • ​Contacts parents/guardians and/or Resource Coordinator to inform them of person(s) supported weekly progress and any incident that may occur at a minimum of 1 time per week. 

    • ​Performs other duties as assigned by Program Manager and/or the Director of Group Supports. 

    ​ 

    ​Record Keeping & Documentation 

    • ​Accurately documents and turns in daily activity and attendance sheets. 

    • ​Incident Reporting – Immediately documents and reports any incident involving person(s) supported and/or staff on CMS and to your supervisor. Ensures proper forms (such as progress notes) are completed and forwarded to appropriate personnel. 

    • ​Appropriately documents person(s) supported activity according to IP.  

    • ​Maintains monthly report and timely submit them to the Program Manager/Supervisor by the 5th of every month. 

    • ​Monitors the safe keeping of sensitive and confidential information and materials. 

    • ​Records and reports all maintenance issues to your supervisor. 

    ​Transportation 

    • ​Transports person(s) supported to and from daily recreational activities in a safe and timely manner.
